1. Your phone number
2. The badge is for ICS membership: cost each $11 plus tax. 
3. The exact name to be printed on each badge
4. Pick up is free, mailing is an additional $10.00 per order. 

Name tags must be paid in advance. 

Email order requests will receive an invoice from our QuickBooks system, that can be paid online. If you prefer to pay by phone please call us at 239-774-2410.
